Please read the detailed instructions on this form carefully and bring it (along with any relevant information, i.e. a copy of the syllabus, course catalog description, often available online at the university’s web site etc.) when you come to see me during my walk-in advising hours (no appt necessary).

Upon completion of external work (=through a course worth at least 2.5 semester credits, taken at an accredited university) please also arrange to have an official transcript (indicating that you passed the course with a grade of C or higher) mailed to the Student Records Office of your college.
